How do employees get their health-care coverage from employers in the U.S.?

I'm confused about employment-based health coverage in the United States.

Do the employers deduct some of the employee's earnings and puts it into funding health care coverage for their employees?

Or do they use the employer's own earnings to pay for the employees' health care?

What does Wal-mart (being a large employer in the U.S.) do?

Most of them buy a health insurance policy for all the employees, from an insurance company. They pay at least half the cost, and the employee pays the rest, with payroll deduction. The employer gets to write off, on their taxable income, the amount they pay for health insurance.
